Survey – NYC Faith-based Non-Profits

Dear Faith-based Non-Profit Leader,

The Department of Health and Mental Hygiene (DOHMH) has contracted New York Disaster Interfaith Services (NYDIS) to help assess and improve the New York City Faith-based Sector’s disaster and public health emergency readiness. You are encouraged to participate by taking this survey, as it will allow us to understand the capabilities and needs of faith-based non-profit organizations (congregations and religious bodies are being surveyed separately).

If you require technical assistance to complete the survey, please contact Peter Mosur at 646-663-1848.

We are asking that you, as the leader of a faith-based non-profit, or your designee, complete a survey about your organization. Your participation in this assessment is both greatly appreciated and essential to improving disaster and public health emergency readiness for New York City’s faith communities and the individuals and communities they serve.

The survey should take approximately 20 minutes. We encourage you, or another leader with direct operational knowledge of your organization, to complete and submit the survey. Each survey and individual answer will give NYC DOHMH and Emergency Management the critical information needed to best prepare and care for all New Yorkers in times of crisis.

We at NYDIS look forward to sharing the results of this survey with you, and continuing our fifteen-plus years of service assisting New York City faith communities’ efforts to prepare for and respond to disasters and public health emergencies.
